Nobody shows up to work to have a bad day. But that's exactly what happened last month when the HMNZS Manawanui grounded and sunk off the South West Coast of Samoa last month as a result of human error. At a press conference this afternoon, the Chief of Navy revealed the interm findings of a Court of Inquiry into the incident which saw 75 people evacuated and the ensuing slow release of 200,000 litres of diesel into the ocean.
